Step 2: Check your email and/or texts for another link to complete your New Practice Member Paperwork.
*If this appointment is for a Pediatric Practice Member, please let us know so we can send you the appropriate paperwork!*
We reserve 60 minutes in the office for your appointment to complete your full health history, consultation, and evaluation. We require that these forms be submitted ahead of time.
If intake forms are not submitted before your appointment, we may have to reschedule your visit to a different time. If you are unable to complete these forms, please arrive 20 minutes early for your appointment.
Step 3: Add your appointment to your calendar!
All initial appointments happen at our Wauwatosa location. Once you become an established Practice Member, we have both the Wauwatosa and Delafield locations in which to serve you.
